• You are eligible for the Lifetime Promise Agreement if you have had Laser Vision Correction originally performed by Richard Levinson, MD.
• You have completed the course of post-operative care and visits that were prescribed by Dr. Levinson. Post-operative visits are typically (but are subject to change)
  

  LASIK 1 day 1 wk 1-2 mos 6 mos 12 mos

  PRK 5 day 2-3 wks 2-3 mos 6 mos 9-12 mos

• Following completion of all your prescribed post-operative treatment, you will have annual comprehensive eye examinations with Dr. Levinson. You are responsible for the annual fees. Dr. Levinson is happy to bill your insurance if he is a participating provider, otherwise he offers a 25% discount off his customary fees.

• If any refractive condition of .75 diopters or greater exists or develops after your procedure, this may be enhanced free of charge, provided Dr. Levinson agrees that an enhancement is medically safe, appropriate and has a high probability of correcting your vision.
  

Exclusions...

There are certain exceptions and conditions when enhancements will not be advised and will not be covered by the Lifetime Promise Agreement.

• Patients who fail to comply with the guidelines and recommended course of follow-up and annual examinations as described above.
• Patients whose decreased vision results from trauma.

• Patients whose cornea is too thin for successful treatment.

• Patients whose vision is reduced due to irregular astigmatism.

• Patients who had RK, ALK, PKP, or any other previous vision correction with a surgeon other than Dr. Levinson.

• Patients diagnosed with any medical or ocular diseases, (cataracts, diabetic retinopathy, glaucoma, etc.) which would compromise a successful treatment.

• Patients seeking an enhancement as a result of presbyopia (a normal aging process in which the lens of the eye loses its elasticity).

Dr. Levinson promises to provide you with free enhancements with six months of post-operative care, provided that you have adhered to the above mentioned eligibility/exclusions and provided that Dr. Levinson is still in practice and performing Laser Vision Correction.
